We respond within one business day
Use the form below for general questions, billing clarifications, or feedback about your visit. Clinical emergencies require immediate local emergency services; this form is not monitored minute by minute.
For appointment changes, include your preferred clinician and any time windows that work. Attachments are not accepted through this form—bring imaging discs or lab PDFs to your visit or upload through the secure member portal after enrollment.
Messages received after five in the evening are answered the next clinic day.
Other ways we help
New members can review pricing plans online before the first visit. Existing members should use the portal for refill requests so pharmacists receive structured orders. Media inquiries may reference our blog for accredited clinical commentary.
We welcome suggestions about accessibility. If a format on this site does not work with your assistive technology, describe the issue in your message and our digital team will prioritize a fix.
What to include in your message
Member ID or the email on your account helps us locate records quickly. Describe whether your question is clinical, billing-related, or technical so we route it to the right team the first time. Do not include passwords or full identification numbers in this form— staff will verify identity through the portal when sensitive actions are needed.
Response times average one business day for non-urgent topics. During seasonal peaks we post banner updates if queues extend to two days. Urgent symptoms always require local emergency services rather than waiting for email replies.
Partnerships and media
Community organizations interested in workshop space or screening events should outline proposed dates and audiences. Journalists may request comment on public health topics; include deadlines and embargo details so our communications team can coordinate clinician availability.
Vendor inquiries about non-clinical products are reviewed quarterly. Clinical supplies and software must meet security assessments before pilot use with member data.
Privacy reminder
Email is convenient but not as secure as the member portal. Avoid sending detailed symptoms or images here if you already have portal access. Read our privacy page for descriptions of how we store and delete marketing versus clinical communications.
Office hours for non-clinical teams
Patient relations and billing respond Monday through Friday from eight in the morning until six in the evening. Messages received on weekends queue for Monday unless marked urgent regarding access to life-sustaining medications— those should also trigger a portal alert if you are enrolled.
Holiday closures post on the home page two weeks ahead. Emergency care remains available through local emergency services even when our administrative lines are quiet.
Translation services
Written correspondence can be provided in several languages upon request. Note your preferred language in the message field so coordinators assign the right reviewer without delaying your answer.
Records and forms
Requests for chart copies or employer fitness forms start here with the date range needed. We confirm identity before releasing documents and deliver through the portal whenever possible. Allow up to ten business days for large exports that include imaging summaries.
